Athletic Forms

From this page you may access the PSAL website for necessary documents - Interscholastic Athletics Parental Consent Form, Medical Form, which are necessary if you plan on trying out for a team during the active school year.

 

NOTE: The Interscholastic Athletics Parental Consent Form and Medical Form, must be filled out between July 1, and the beginning of school in September for the upcoming academic school year. Completing these forms (which requires a visit to and dated signature of a doctor) prior to this time period will disable a student from trying out for an athletics activity that fall.
